Step 2: Complete this Form - Parents must attend The Reach when signing up under 18s.

Step 3: Once you have completed your Registration Form, please go to Reception Desk to finish the process. If you DO NOT go to Reception your Registration is incomplete.

INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later.
A PARENT OR GUARDIAN MUST ATTEND THE REACH ON THE FIRST VISIT TO COMPLETE THE REGISTRATION PROCESS.

IMPORTANT!
1) Waivers for minors must be signed by a PARENT or GUARDIAN only.

2) Only fill out a form for one person by selecting either 1 Junior or 1 Adult
